投稿標題(英): Characterization of the coagulase-negative staphylococcal isolates from blood cultures
投稿摘要: Background Coagulase-negative staphylococci (CoNS) be considered as contaminants during blood sampling procedures unless isolated from more than two positive blood cultures less than 24 hours apart. The contamination rates also be monitored as quality indicator of insufficient disinfection of the skin. Previous studies discovered that the CoNS are the common cause of nosocomial bloodstream infections. But, the identification of CoNS species are not routinely performed in laboratories. We would like to investigate the variations of contaminanted CoNS by MALDI-TOF (matrix-assisted laser desorption ionization-time of flight). Methods All blood culture positive samples confirmed as CoNS by MALDI-TOF or conventional biochemical tests. We used computer-assisted management programs SAS to evaluate the raw data to monitor routinely the ratio of CoNS infection. We analyzed samples from positive blood cultures for CoNS from May 2020 to April 2021. Results At first, the proportion of patients with positive blood cultures of all blood culture specimens in our hospital were 10.7% in 2020. The overall contamination rate of blood culture were 2.4% in 2020. Meanwhile, 627 (74.6%) out of 840 blood cultures with CoNS from the emergency department. We demonstrated that Staphylococcus epidermidis (n=231, 27.5%) is the predominant species, followed by Staphylococcus hominis (n=203, 24.2%) and Staphylococcus capitis (n=158, 18.8%). Interestingly, the isolation rates of Staphylococcus hominis were significantly higher in female more than male (p<0.05). Moreover, various CoNS have different antibiotic susceptibility testing results in our antibiograms. Conclusion In summary, our data analysis revealed that the potential and convincing morbidity of the first isolated CoNS help clinicians make effective clinical decision. The continuous surveillance of CoNS identification based on MALDI-TOF should be performed to improve patient safety because of the treatment regimes should be individualized for different isolates.
