Study on optimally utilizing resources when caring for patients with COVID-19 at an isolation ward using simulation software

Objectives: The COVID-19 pandemic has created a public health crisis. It is an important issue for hospital administration and infection control to coordinate all resources and manpower. Simulation model was built and validated to address this challenge.

Methods: A simulation model was built by the software FlexSim Healthcare® using data of an isolation ward in a north Taiwan regional hospital. Data collection and modeling were executed based on historical data (patients' arrival, manpower, resources) by the isolation ward, from May 9th 2021 to May 31st 2021. The number of beds, and resources(gloves, gowns, N95 face masks, personal protective equipment) for each shift was determined. Detailed process mapping with all micro-processes was done.

Results: During the study period, there was 57 patients with COVID-19 admitted to the isolation ward. Among them, there were 23 patients with mild illness, and 34 patients with severe illness. The utilization of doctor, nurse practitioner, nurse, respiratory therapist, and cleaner was 55.85%, 59.27%, 88.72%, 63.88%, and 58.04%. The consumption of N95 face mask, gloves, gowns and protective clothing was 1272, 8122, 6850, and 1272.

Conclusions: The results showed insufficient nursing staff during the most of time and cleaning staff in few days of study period. Simulation helped to identify the shortage of manpower and optimally utilize resources during a pandemic of an infectious disease.
