The Effectiveness of Implementing Multi-strategy in Education to Improve Hand Hygiene Practices among Inpatient Caregivers

Background
Caregivers are an important provider of daily living care for multidrug-resistant organism (MDRO) inpatients in chest medicine ward; they are at risk for contracting and spreading MDRO from frequent interactions with patients. Improving the hand hygiene (HH) compliance of caregivers has important significance in reducing the incidence of infection. However, we have little information about HH compliance among caregivers of MDRO inpatients in most medical institutions. Therefore, we assessed the effect of multimodal interventions on improvement of HH compliance.
Methods
This project was conducted in chest medicine ward from May 2023 to October 2023. A total of 64 caregivers, were included in the HH audit. The audit was analyzed by direct observation and by a completed knowledge, attitude, and practice (KAP) questionnaire. Using the WHO’s Hand Hygiene Observation Tool, we surveyed a total of 370 HH opportunities.
Results
Overall hand hygiene behavior compliance rate was significantly increased from 37.2% to 81.8%. Questionnaire-based data were also analyzed to assess KAP of HH. We found that only 52.2% of caregivers were aware of the WHO’s Hand Hygiene. In the post-intervention phase, we observed a significant improvement in KAP. The most frequent moment for HH was “after a touching patient” (88.2%) and the lowest compliance was “before clean/aseptic procedures” (78.6%). Compliance during “visiting” was highest (86.8%), with hands being less frequently washed during “resting” (67.9%).
Conclusion
This project provides valuable information on caregiver HH knowledge and behavior, and our findings emphasize the need for multifaceted HH education incorporating caregivers’ needs and engaging them as essential infection prevention partners for inpatients.
