Gleaming Insights into Cleanliness: A Novel Perspective on Infection Risk Warning Using ATP Bioluminescence Detection

Environmental cleanliness is a critical aspect of infection prevention in healthcare institutions. With advancements in infection control technology, the ATP bioluminescence detection method has emerged as a new tool for evaluating cleaning quality. In 2021, a regional hospital in southern Taiwan implemented this method to verify the quality of environmental cleaning. Routine monitoring involved sampling five points in general wards and ten points in intensive care units (ICUs). The study revealed that ATP bioluminescence readings exceeding standard thresholds (general wards >50 RLU, ICUs >25 RLU) were associated with increased infection rates. In one instance, the highest recorded value reached 6654 RLU, exceeding the threshold for general wards by over 133 times. This study aims to explore the use of ATP readings for early warnings and interventions to mitigate infection risks effectively.

This study employed unannounced monitoring of general wards and ICUs, with routine sampling conducted at five points in general wards and ten points in ICUs. ATP bioluminescence readings were recorded, and their correlation with infection rates was analyzed. When abnormally high readings, such as 6654 RLU, were detected, an investigation was initiated to review the cleaning procedures. The environmental cleaning staff were found to have deviated from standard operating procedures, including using incorrect methods such as direct spray disinfection, not washing cleaning cloths, and failing to replace cloths throughout the cleaning process. Targeted corrective actions were implemented, including staff training, process restandardization, and subsequent follow-up. Data were statistically analyzed to assess the relationship between ATP readings and infection rates.

The results demonstrated a significant increase in infection rates (p<0.05) when ATP readings exceeded the standard thresholds (general wards >50 RLU, ICUs >25 RLU). During monitoring, an abnormally high value of 6654 RLU was recorded. Further investigation revealed that improper cleaning procedures, such as using spray disinfection and failing to clean or replace cleaning cloths, contributed to poor environmental hygiene. After intervention, ATP readings and infection rates decreased significantly. The 6654 RLU site returned to standard ranges following corrective actions, including enhanced training and standardization of cleaning protocols.

The ATP bioluminescence detection method provides a novel perspective on environmental cleaning verification and infection risk warning. Its integration into routine monitoring and data analysis enables the timely identification of environmental cleaning deficiencies and effective intervention. This study confirmed that even in extreme cases, such as ATP readings exceeding standard thresholds by 133 times, swift corrective actions can significantly reduce infection risks and improve environmental cleanliness. Routine sampling in general wards and ICUs demonstrated the practicality and reliability of this method, offering robust support for hospital infection control efforts.
